GAD-2 & PHQ-2
Mental Health Pre-Screener
Instructions: Over the last 2 weeks, how often have you been bothered by the following problems?
Over the last 2 weeks...
Not at all (0) | Several days (1) | More than half the days (2) | Nearly every day (3)
Anxiety (GAD-2)
1. Feeling nervous, anxious, or on edge
2. Not being able to stop or control worrying
Mood (PHQ-2)
1. Little interest or pleasure in doing things
2. Feeling down, depressed, or hopeless
How to Score
Add the scores for the two Anxiety questions (GAD-2) and then separately add the scores for the two Mood questions (PHQ-2). Each section is scored from 0 to 6.
What Your Score Means
Score of 3 or higher (on either section): This is considered a "positive" screen. It suggests that symptoms of anxiety or depression may be impacting your life.
Next Step: If you score a 3+, we recommend scrolling down on the screeners page to take the full GAD-7 or PHQ-9 for a more detailed look at what’s going on.
A Note from Vibe: This is the Check Engine light for your mental health. It doesn't tell you exactly what’s going on under the hood, but it tells you it's time to pull over and take a look.
Important: If you are in immediate distress or having thoughts of hurting yourself, please reach out for support. You are not alone, and help is available 24/7.
Call or Text: Dial 988 to reach the Suicide & Crisis Lifeline.
Crisis Text Line: Text HOME to 741741.
Emergency: Call 911 or go to your nearest emergency room.
